We help people stay in their home and this includes older people who need support. Our services cover areas such as preparing meals, medication services, shopping, collecting benefits, respite and end-of-life care.

Personal Care

Designed to support you with your everyday activities, our personal care service is tailored to your individual needs. This includes help with morning and evening routines, bathing, dressing, continence care, medication prompts and meal preparation.

Complex Care

People with complex and continuing care needs often need the help of highly skilled healthcare professionals and support workers. They have specific skills which are personalised for you.

Children and Families

As the organisation has grown, Apasen Home and Community Services has been able to expand the range of specialist services it offers. We support vulnerable families to make sure that their children are able to grow up in an environment where they are well looked after. Examples include helping children with a disability with their personal care and to get ready for school.

Support for People with

Learning Difficulties Apasen Home and Community Services supports people with learning difficulties every day to achieve greater independence and fulfilment. We operate contracts specialising in supporting children and adults with learning disabilities. Our trained support staff provide services from personal care to escorting service users to and from day-centres or clubs. This gives respite to parents and carers living with the daily challenges of caring for a child or adult with challenging behaviour.

Medication Services

Whether you need a reminder to take your medication or help to take it, we can provide you with a care assistant who can help. Our care assistants are trained in Medication Administration (Level 2 & 3).

End-of-Life Care

End-of-Life Care is a specialist area of healthcare for people with life-threatening illnesses. Our End-of-Life Care Service can be there for you at all possible stages of illness, including pre-diagnosis, diagnosis,treatment, cure, death and bereavement. We want to achieve the best quality of life for those suffering from a progressive illness and for their families

Meal Preparation

Whether you need help with heating up a meal, preparing a snack or cooking your favourite home-cooked meals, we can provide a worker who will suit your cultural needs and preferences.

Care for the Elderly

Our care staff visit elderly people in their homes to help with essential daily living tasks, ranging from shopping and cleaning to washing, changing sanitary wear and using the toilet. Typically, a care assistant will visit several people in succession every morning (and perhaps, also lunchtime or evening) and help them to get up and prepare for the day. Once or twice a week, they may shop and collect benefits on their behalf.

Hospital to Home

Returning home after a hospital stay can seem daunting. Our Hospital-to-Home Service eases this transition and supports people as they settle back into their family surroundings. Whether you need a high level of care or just help with everyday activities, we can support you as soon as you leave hospital. We will work together with your hospital discharge team and occupational therapist to make sure you recover faster.

Support in the Community

Would you like someone to come with you on an outing or taking part in an activity? We can provide you with a worker to support you in visiting your local shops, in a walk in the park or for a day out at your favourite place.

Social Companionship

Our Social Companionship Service is most suited to those who are reassured by having someone there with them. Whether you need help with household chores, going for a walk or just someone to sit, chat and read with you, we can provide support.

Mental Health Support

Although most of our work is with children and people with learning disabilities, we also support people with mental health problems who are living in the community. Typically, our support workers will help people with aspects of their daily living that they may find challenging or even impossible. This may mean accompanying a person to a day centre or classes, or helping with shopping, cleaning or washing clothes. Shifts tend to be longer than for elderly personal care..

Domestic Services

We provide staff to help with tasks such as housework or shopping. This can be as a standalone service to the elderly or disabled or as a supplementary service as part of a larger package of personal care..

Holiday Care

We know that there is nothing quite like a holiday to recharge your batteries. And if you need regular care, you can still have a break. As part of our Holiday Care Service, a support worker can come with you on your holiday. This means both you and your family can relax, safe in the knowledge that you are getting the personal attention and support you need to get the most out of your break.
